Paul and Diana have kindly allowed their most recent registration form to be shared :) I've uploaded it to the armageddon.cosplay@gmail.com docs so anyone with the link can see it:
Team Naruto Auckland 2010.

And to create the form there were notes made along the way that were then put into a template and then photos added (and then converted to pdf- if anyone wants to do this for free use open office- I do and it is magic :) )

Do not think you have to fill in your forms in so much detail!!! This is the extreme end. I wanted to share as I think it is a really valuable document in terms of showing how to work on a project over time and for showing all that hidden detail that the audience can't see.
